Bolton Family Clinic

We provide a Nurse Practitioner who manages care of individuals from 2 weeks old thru old age. Bolton Family Clinic is a family practice clinic which offers primary care to the community. Our mission is to offer excellent quality primary care and health education to every individual in our community while providing this care in a culturally sensitive, professional and compassionate manner with special emphasis on reaching the underserved. We accept Appointments as well as Walk-In patients. The Bolton Family Clinic provides preventive health services, counseling, well person check-ups, EPSDT screenings, and many other services designed to deliver health care to the entire family. We are staffed by Certified Nurse Practitioners and a Physician. We accept private pay, Medicare, Medicaid, and certain insurances-call for more information or an appointment

Nursing Home Practice

We provide a Nurse Practitioner who is scheduled at select Nursing Homes. We can manage acute problems as well as manage skin issues (including pressure ulcers) and/or assist with making routine rounds alternating with the primary physician. We will work to enhance the care the primary physician provides while decreasing liability to the Nursing Home as well as the Physician and Medical Director.
